1. Locke meant in time, and I also think the sequence at the cabin with claire/christian occured before the doctor washed up on shore, and that the island has already moved, when we see the doctor wash up on shore.
2. I think the island will keep Michael alive until the message changes from "Not Yet" to "Now", then he starts the bomb, and the boat, and everyone on it explode.
3. Claire is alive.
