No form of birth control (aside from abstinence) is 100% effective. A person should always be aware of the fact that any act of intercourse could result in a pregnancy. That's just one of the realities of life - even if you are taking proper precautions, there is still a pregnancy risk involved.
(Doesn't change the fact that what these girls did was reprehensible, of course.)
