The problem being is you're getting an opinion of living under one system but not the other, so you're left with their impression of what the system is, and its commonly positioned as "show your credit card to the ambulance driver or be abandoned" by defenders of the NHS.
I bet they had health insurance for their US visit!

EXACTLY...We're currently not a "community of nations".
We are individual nations that may or may not have cultural, ethnic, ideological or religious similarities.
While we've shared strong civilizational ties over the past two to three centuries, the past seventy odd years have demonstrated a particular zenith in our collective relations...
I'm guessing that changes in the not too distant future (10-15 years). And NOT because our erstwhile NATO aliies choose to disassociate with US, but because WE realize that said allies are no longer allies regarding culture, ethnicity or religion. And therefore, most importantly, about ideology.
Amsterdam in 2030 is a totally different place than today. Ditto Rotterdam, Brussels, Marseilles, Strasburg, Paris, Berlin, Duesseldorf, Cologne, Hamburg, Sheffield, Birmingham, London and whatnot.
